This should be filled out by the person RECEIVING the accessory or barrel (not shipping it).

It may take us up to 5 days to send our paperwork to your seller.  If you haven't seen a confirmation by that time please follow-up with us.

We need this filled out in order to properly match up your order with the shipments received. Please include the following in the comments:

2. What item you ordered
3. How many of the item you ordered
4. Order number if you have it
5. If from GunsAmerica, Gunbroker, or Gun Auction - provide item or auction number

If you are receiving the barrel or accessory from a private party make sure they complete a form similar to this Personal Sale Statement and include it with the package. Failure to include this from a California seller means we will need to charge you sales tax on the transaction.
